What This Privacy Policy Covers
- This policy covers how The1Form treats personal information that is collected and received on and through our website ands service. Personal information is defined as your name, address, phone number, health records and other personally-identifiable information, including information related to any past use of The1Form services.
- This policy applies solely to information collected and received by The1Form and does not apply to the practices of companies that we do not own or control, or to people that we do not employ or manage.
Information Collection and Use
General
- We collect personal information when you register with The1Form.
- When you register we ask for information such as your name, email address, birth date, gender, and zip code. We may also ask for your address, Social Security number, and other important health-related information. Once you register with The1Form.com and sign in to our services, you are not anonymous to us. We will also collect billing and payment information from you. We will use such billing and payment information solely to process your payment(s).
- Our member organizations (Gym Managers, Competition Companies, Event Coordinators and the like (referred to herein as Member Organizations)) may provide specific medical and other forms for registration and/or request certain information for registration with the Member Organization.
- The1Form automatically receives and records information on our server logs from your browser, including your IP address, The1Form cookie information, and the page you request.
- The1Form uses information for the following general purposes: to fulfill your requests for services, improve our services, and to contact you.
- We may aggregate non-personally identifiable information and statistical data about use of our services. We may use non-identifying and aggregate information to better design our services and our website. We may provide such aggregated and statistical data to third parties for such purposes as we deem, in our discretion, appropriate, including to third parties to assist them in understanding usage patterns for certain content, services, advertisements, promotions or functionality that may be available on our website.
- We reserve the right to send you certain communications relating to The1Form service, such as service announcements and administrative messages, that are considered part of your account, without offering you the opportunity to opt-out of receiving them.
Children
- Children under age 18 may not register or use The1Form. Minors must be registered under a parent or guardian's account. * We will not contact children under age 18 about our services or solicit information from them.
Information Sharing and Disclosure
- We do not rent, sell, or share personal information about you or your child with individuals or nonaffiliated companies except to provide services you've requested with your permission, or under the following circumstances:
- We may share personal information with third party vendors, consultants and other service providers who are engaged by or working with us in connection with the operation of The1Form and who need access to such information to carry out their work for us. For example, we may use credit card processing companies to process payments. We require that our service providers use any personal information we provide only to carry out their work for us.
- We respond to subpoenas, court orders, or legal process, or to establish or exercise our legal rights or defend against legal claims.
- We believe it is necessary to share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of The1Form terms of use, or as otherwise required by law.
- If The1Form is acquired by or merges with another company, we may transfer or assign your personal information as part of such merger or acquisition. However, we will use good faith efforts to notify you before information about you is transferred and becomes subject to a different privacy policy.
- We may provide personally identifiable information to our subsidiaries, affiliated companies and service providers for the purpose of providing and maintaining our service and to allow such parties to contact you regarding available products or services.
Cookies
- The1Form may set and access The1Form cookies on your computer. A cookie is a small text file containing a string of alphanumeric characters which will uniquely identify your browser when you visit our site. We will not link any cookie information with your personal information.
Your Ability to Edit and Delete Your Account Information and Preferences
General
- You can edit your The1Form Account Information and update your medical and health forms at any time.
- You can delete your The1Form account by visiting our Account Deletion page.
Children
- Parents can review, edit, and delete information relating to their child's The1Form account.
- If a parent chooses not to allow us to further collect or use a child's information, parents enrolled in a The1Form account can delete their child's account by contacting The1Form.com at (866)-975-9888. Please note however that any information that was previously provided to our Member Organizations may continue to be held by such parties in their own files.
Confidentiality and Security
- We limit access to personal information about you to employees who we believe reasonably need to come into contact with that information to provide services to you or in order to do their jobs.
Who Has Access?
- Parents have access to information of their minor children in The1Form.
- We will share your information as necessary with Member Organizations in connection with your registration for events and competitions. For example, gym managers have access to all of the information for all the children who are enrolled in their gym during and after the time the child is a participant. Competition Companies have access to all of the information such as forms and vital health information of their affiliated gyms and the athlete participants that belong to those gyms. Access will only be granted if the gym and athlete is competing in an event put on by the competition company.
- We require that the Member Organizations authorized to access your personal information use the personal information only as required for the athlete's participation in the event and as permitted under applicable law. However, we do not control how such entities use, store or disclose such information.
Third Party Sites.
- The1Form website may contain links to other sites, including sites operated by Member Organizations and other clubs, teams, organizations or other persons. Our Privacy Policy does not apply to any personal information which you may provide on or through these third party sites. We are not responsible for and do not control, monitor or endorse the privacy policies or practices of such third party sites. You should carefully review the privacy policies and other terms for third party sites prior to using such sites.
Changes to this Privacy Policy
- We reserve the right to update this policy and will notify you about significant changes in the way we treat personal information by sending a notice to the primary email address specified in your The1Form account or by placing a prominent notice on our site.
