A signed form credit is good for one signed form, so you just buy
the amount that you need and receive discounts for purchasing in
bulk. A credit is valid for 12 months from the date of purchase.
If you buy 500 credits you are expecting a form to be signed by
500 participants within the next 12 months. You only need to
purchase credits for forms that you create.
Do my credits expire?
Yes. Credits are valid for 12 months from the date of purchase.
If you run out of credits, parents will no longer be able to sign
your form until you purchase more credits. However, you will
always have access to signed forms that are listed in your
account.
Do parents have to pay to sign forms?
No. Credits are purchased by groups and events. It is FREE for
parents to sign event forms and group forms.
Does it cost anything to create an account?
No. Events, groups, and parents can all create accounts for FREE
at The1Form. Our flexible pay-as-you-go pricing model is only for
signed forms. Events and groups purchase credits to create and
manage medical release forms. All new parent accounts are
required to complete our one-time verification process.
No. If an event invites a group to sign their forms using
The1Form, it is FREE for the group. You will only need to
purchase credits if you want to create and manage a separate
medical release form for your group.
Do parents have to sign my form for each event and will I need a
credit for each event?
No. The1Form guarantees that parents only have to sign a form one
time! You can use that signed form for one event or 3,000 events
and it is still just counts as one credit.

Do I have to pay up front or are there payment options?
Our flexible pay-as-you-go payment system allows you to choose
the amount of credits that you need. Purchasing in bulk allows
you to receive volume discounts. If you have a form that needs to
be signed by 10,000 participants over the next 12 months you can
purchase 10,000 credits at one time. You can also choose to
purchase credits 1,000 at a time throughout the year. In this
example, purchasing 10,000 in bulk will save you money with the

What is the difference between an event and group account?
Examples of events include camps, competitions, clinics,
tournaments and leagues. Multiple groups attend each event. With
an event account, you invite groups to sign your required medical
release forms online with The1Form. Examples of groups include
gyms, teams, schools and clubs. Groups invite parents to sign
their medical release forms. Groups also invite parents to sign
medical release forms for events that their participants attend.
All you need is an Internet connection. No additional software is
required. The1Form is optimal on the following browsers: Internet
Explorer 6.0 SP2 or 7.0, Firefox 1.5 and up, Safari 1.2 and up.

How do I cancel my account?
Our flexible pay-as-you-go credit system allows you to use
The1Form with no long term contracts. Credits are pre-purchased
and deducted from your account each time the form is signed. Once
forms are signed you have access to each form in your secure
account. If for some reason you would like to cancel your
account, please call customer service and we will shut down
access to your account. No reimbursement or refund will be given
for purchased credits.
