Question: How does The1Form work?
The1Form allows organizations requiring medical release and liability forms to host them on our site, accessed by a designated key to their forms. As a parent of a child participating in that organization, you simply need the access key and an account to sign all your child's forms. If your organization is not currently using The1Form then make sure you share the news and get them signed up today.
Do I have to purchase an account for every child?
Yes. A new account must be purchased for each individual child. This assures that the parent or guardian verifies their identity for each child.

What are the system requirements?
All you need is an internet connection. No additional software is required. The1Form is optimal on the following browsers: Internet Explorer 6.0 SP2 or 7.0, Firefox 1.5 and up, Safari 1.2 and up.
Is The1Form secure?
Absolutely, The1Form's parent company, Magnus Health Portal is a Personal Medical Information provider that makes security its Number 1 Priority. To begin with The1Form is hosted from a SAS 70 type II certified data center to ensure realibility and security. Access to the The1form is protected at the application level by a unique name and password combination. Transmissions between the Member's browser and our secure servers are encrypted using Secure Sockets Layer (SSL) and our data center is equipped with state-of-the-art hardware to protect against emerging threats. All transactions are secured with 128-bit encryption and all data is protected behind redundant firewalls.
How do I cancel my plan?
The1form membership is good for 1 year. At the end of each year you will have to renew your membership and verify your identity. If for some reason you would like to cancel a membership within that year, please call customer service and we will shut down access to your account. No reimbursement will be given as this is a one time verification payment and not a subscription fee.
